    Hi Lifeline… My name is Kim Molina, I first obtained a phone about 2 or 3 years ago the phone stopped working it was going to cost 25.00 to replace it under ensure I believe. I then obtained another phone the carrier was access, well it wasnt more than one month and there was a descrepency between having signed originally with ensure (or something similiar to ensure) and signing up with access I tried cancelling out of the first unsuccessfully I guess because the cool pad device that I had received was deactivated although I did LOVE the phone..forgive me its a little lengthy but I want to be forthcoming with all the info. I received another phone lost it after the fire in my apartment it was still working someone may have gotten ahold of it but then recently received another phone and again it was turned off after one month. I had used it although.. well shortly after I received a pink letter from lifeline explaining that they need the SSI information from me since I had used my Social Security Benefits letter to qualify the problem was that I have SSA not SSI and previous qualifications were for housing or Section 8
    always with some type of validation follow up that I was responsible for providing to continue service when I had already been approved at the time I received the phone. Now this last attempt to be graced with your generousity the people generating the SafeLInk phone after applying informed me that I would be receiving the phone by mail in one week. Its been a month and I was reading the pamphlet they provided me with and Inrollment ID 10167 instructing me to follow up in 72 hours to check the status of application. Knowing that there would be obvious and previous phone that I probably would be waiting for ever and there is no contact number to be informed by Lifeline which is why I inquiring now to you and hope this is the correct dept to be approaching with my concern. I have within the last ten years have had an enormous change in life and lifestyle I cannot express to you how wonderful you are by providing this service and eliminating the expense of having a phone let alone the ability to access the internet and THANK YOU. I hope to be reinstated for your service and I now have Medical and or Calfresh (food stamp) to qualify. I dont have a phone now or internet service or access to internet at home I am at a One Stop now, so again my name is Kim M. Molina I live at 494 H. street Brawley CA 92227. I guess I can wait for some sort of acknowlegdment letter from you and hopefully a phone. God Bless and thank you for your time and consideration of this matter.
